Well, the tag names are case insensitive so it is just a matter of taste. My taste says lowercase looks less "yellish" and is easier to read. :-)

I believe the quotes are supposed to be there around all attributes, even though most, if not all, browsers accept and understand them without.

Another case of personal opinions. The standard says it doesn't matter.

Yes they have. HTML is a standard, and you can find all sorts of info at their site at http://www.w3.org/MarkUp/.
